I would like to have a SETTITLE command to be able to set the title of the
window asking for the PIN.

I provide a patch to add this command to pinentry/pinentry.{c,h} and
gtk+-2/pinentry-gtk-2.c.
Adding support of this command to the gtk and qt programs should be easy.
