Add DEFAULT_ALL_SMIME install parameter

* src/inst-kleopatra.nsi: Ignore a registry value.
* src/make-msi.pl: Add new comonent for file extensions.

This allows users that use GnuPG VS-Desktop a lot with
SMIME to take over the File extensions Windows uses
for certificates.
