Description

Hi, "Current Action" commands are not updated for external actions.

4 Add to "tool bar"
5 Now open the toolbar configuration menu, the item you added does not appear.

Interesting. I didn't even know that this feature exists.

Two more observations:

If you restart Kleopatra after adding an action to the toolbar then it's shown correctly in the toolbar configuration. So this is an update problem of the toolbar configuration.
The same problem exists in Okular. This means that this seems to be a general problem that affects many KDE apps.
